Fall 2009 Registration and Cost

Cost:

Thanks to the generosity of many, attendance at Camp Hope is offered free to all children.

Donations are welcomed to help cover the costs of running Camp Hope.

Registration:

In order to register your child to attend Camp Hope please print, complete and return the following forms.

Once all forms are received, you will be contacted by one of the Camp Directors to verify that Camp Hope would be a good fit for your family. Following this screening, you will be formally added to our camper list and additional camp information will be provided (e.g. maps, what to bring, etc).

  1. Camp Hope Registration Form

  2. Consent to Medical Diagnoses and Treatment, Release of Medical Information, and Child Abuse Reporting Requirements

  3. Camper Health History

  4. Authorization and Consent to Photograph and Publish Photographs

  5. Camp Hope Release of Liability, Waiver and Assumption of Risk Agreement

  6. Taylor Family Foundation’s Camp Arroyo Release of Liability Form

We must receive all 6 completed forms as paper ("hard") copies.
